The City of Sapporo offers health check-up for children who are going to enter elementary school in April 2024.
Those who are eligible (born between April 2, 2017 and April 1, 2018) must have received an envelope which looks like the photo below.

Each school has different date/time for the check-up.
See the letter enclosed in the envelop or call the Sapporo City Call Center.

Physical check by a doctor, dental checkup, eye test, etc.
[What to bring]
(1) The letter itself
(2) Health Survey and Examination Form (copying paper enclosed in the envelop)
(3) Inner shoes or slippers and a bag to put them in, and a black pen
NOTE: Health Survey must be filled out in advance. Please refer to English translation.
