When you test positive

When you test positive at medical institutions / PCR centers etc.

Register yourself on the [Sapporo COVID-19 Medical Care Judgment Site]

Enter your information such as your name, address, date of birth, the name of the medical institution you consulted, the date on which the symptoms started, and your current health status.
The following is judged by the information you mentioned above.

* This site is in Japanese only. Enter using the translation function of the browsers, or call the Call Center at 011-350-5574 (9: 00 ~ 20: 00 every day) and have them input the details on your behalf. (Call Center supports English, Chinese, and Korean languages through three-way calls)

(1) Mild symptoms / Low risk of aggravation → Home treatment

The end date of treatment is automatically displayed. Observe your health using the app etc. You may also receive phone calls from the health center.
If you feel sick or uneasy, please contact the "Tested Positive Support Center 0570-022-680 (8:00~21:00 every day)".

(2) Severe symptoms / high risk of aggravation → Not home treatment (Accommodation treatment or Hospitalization)

Health center staffs will call you and ask the details. (It may take several days to contact you.)
The doctors at the health center will decide whether hospitalization, treatment in a hotel (accommodation treatment), or  home treatment.

For the following people (1) to (5), the symptoms may suddenly become worse, so enter the information correctly.

(1) Elderly people (65 years old or older)  (2) Those who have diseases such as diabetes, heart disease, respiratory diseases, dialysis etc.  (3) Those who are pregnant  (4) Those who have Immunodeficiency diseases such as HIV or those who take anticancer drugs  (5) Those who have cancer and diseases such as leukemia

* If symptoms get worse by the time the results are known, call the Emergency Medical Consultation Center Sapporo # 7119.

Positive with an Antigen Test Kit

 Those who fall into all of the following categories from ① to ⑦ should register  with "Covid-19 Tested Positive Registration Center".

① Lives in Sapporo
② Those who were tested corona positive with the designated medical antigen test kit and it is within 9 days from the date of symptom occurrence. (The onset date is 0 days).
③ Symptoms are light and stable
④ No malignant tumors or leukemia.
⑤ Not dialyzed
⑥ No immunodeficiency disease  such as HIV or cancer
⑦ Have a smartphone, tablet, or email address

Register with the "Covid-19 Tested Positive Registration Center " (Starts from May 1)

Enter your information such as name, address, date of birth, and your current health condition. And take a picture of the antigen test kit with the positive test result and upload it.

Within 24 hours after registration is completed, you will receive an e-mail from the health center about test result and treatment. Please cooperate in self-restraint until you receive the e-mail.

Those who have symptoms and wish to see a doctor, or those who do not meet one of the above ① to ⑦ and cannot use the registration center will be judged by a medical institution.

Please consult with your home doctor or a medical institution that provides medical care for those who have tested positive by self-test. You can search it from Sapporo City website, or Emergency Medical Consultation Center Sapporo # 7119 can also provide the information. (Reservation is required and some medical fee such as the first medical examination may be your own expense.)

If the symptoms get worse before the notification, please contact the Emergency Medical Consultation Center Sapporo # 7119.

When you are informed by the person who tested positive

1. Ask him/her the following;

・the date on which s/he had symptoms (If no symptoms, the test date)
・check the most recent date on which you met them (to make sure whether you spent time together within 2 days before the test or he/she had symptoms)

2. Monitor your health condition, and if you have symptoms, go to 「When  you feel sick」.

3. If you have NO symptoms, check the possibility of infection.

(1) The last time you met the infected person was at least 3 days before s/he had symptoms (or s/he had a test if no symptoms).
⇒Possibility of infection is low.

(2) The last time you met the person was within 2 days before s/he had symptoms (or s/he had a test if no symptoms.)

A. You talked with the person without a mask for more than 15 minutes within the distance of less than 1m.

⇒Possibility of infection is high.

For 10 days including the date on which you met the infected person (if no symptoms, two days before the test date), do the following and monitor your condition.

・Take body temperature at least twice a day

・Refrain from non-urgent outings including work

・Avoid contact with other people

B.  You DID NOT talk with the person without a mask for more than 15 minutes within the distance of less than 1m.
⇒Possibility of infection is low

 NEW  Special Cash Payment for Households Exempt from Resident Tax

Considering the long-lasting effects of COVID-19, the government has decided to provide a " Special Cash Payment for Households Exempt from Resident Tax" (100,000 yen per household).
Foreign residents who have resident registration in Japan as of December 10, 2021 are eligible to receive this payment if they meet the following conditions.

Be aware of scams and identity theft

  • The government and the Sapporo City  will NEVER ask you to operate an ATM, transfer fees for payment, or send an email asking you to use an URL for this benefit.
    If you have got a suspicious phone call, letter or email , please contact the Consumer Center, a police station or a police headquarter (#9110).
  • Be aware of fake websites designed to look like those of the government or the Sapporo City.  If you see anything suspicious, like unfamiliar characters at the end of the URL, do not open the URL. Do your own search and check the real website.

[Eligible Households]

1. Household exempt from resident tax:

Those who have resident registration in Sapporo as of December 10, 2021 (including households on welfare)

2. Households whose income level has dropped sharply:

Those who have resident registration in Sapporo at the time of application, and whose income level has dropped sharply due to the effect of COVID-19 and are deemed to have the same income level as 1 above.

[How to Apply]

1. Household exempt from resident tax

Sapporo City will send a check sheet (see the picture of envelope) from February 1 onwards. Once you receive it, fill out the necessary part and mail it within three months from the mailing date.

The payment will be made approximately after three weeks from the date your application is received unless there are any mistakes in the document.

2. Households whose income level has dropped sharply

The following procedures are required. If there are no mistakes in the application documents, payment wil be made within one month from the date on which the City receives the application.
The application deadline is Friday, September 30, 2022. *Must be postmarked on or before the deadline.

(1) Call the Special Cash Payment Call Center(050-3172-7731)and tell them that the income of your household has dropped due to the effect of COVID-19 and it is equivalent to the tax exempted household.

(2) During the telephone conversation, the staff will ask you questions about your current situation. The application documents will be sent from the City.

(3) Once you have received the documents, fill out the necessary sections and prepare the following documents.
(A) Certificate of your current income,  (B) Your ID,  and (C) Photocopy of the passbook that shows the account details to which you wish to receive the payment. 

(4) Put necessary documents described in (3) into the return envelope and send it to the City.

For persons who have been evacuated because of DV (e.g. violence by a spouse or other relative)

If you fall under the following conditions and are considered to be either 1. a household exempt from residential tax, or 2. a household whose income level has dropped sharply, you can apply to the municipality where you currently live to receive this benefit. For more details, contact the Special Cash Payment Call Center (050-3172-7731).


(Conditions)
・A protection order has been issued under the Act on the Prevention of Spousal Violence.
・A "Certificate for the Protection of Victims of Spousal Violence" has been issued by a counselling center.
・The resident registration (Jumin-hyo) has been changed to the municipality of residence on or after the day following the reference date, and is subject to "support measures" such as restrictions on access to the basic resident register.

Self-reliance Support Money for People in Need 

Those who cannot borrow more General Support Fund (loans for necessary living expenses) from Social Welfare Council because they finished borrowing the maximum amount and so on and their income and savings are under specified amount may be eligible for “Self-reliance Support Money for People in Need”.
Eligible persons will receive the letter.
To receive the money, continuous job-searching activities (visiting Hellowork etc.) and submission of report to Sapporo City are required every month.

School Expenses Assistance(City of Sapporo)

Assistance for school expenses such as school supplies, lunch, field trip and so on to the households with elementary and junior high schoolers whose income is a below certain amount.

Special Cash Payment for Households with Children

As one of the measures to counter the economic impact of the COVID‑19 crisis, the Japanese government has decided to provide the special cash payment to households raising children from 0 years old to high school students (born on or before April 1, 2006).

1.  Households that do not need to go through the application process (paid on December 27, 2021)

 Eligibility   (Income restrictions apply)
(1) Households that have received child allowance for September 2021 from the Sapporo City; and
(2) Households that are raising children born between April 2, 2003 and April 1, 2006, and certified for child-rearing allowance (as of September 30, 2021).

 Payment date  December 27 (Mon), 2021

 Benefit amount  100,000 yen per child

* There is no procedure to receive money.
* The money has been transferred to the bank account where you receive the child allowance/child-rearing allowance.
 

2. Households that need to go through the application process

 Eligibility     (Income restrictions apply)
(1) Households raising children who were born between April 2, 2003 and April 1, 2006 only (as of September 30, 2021, and except households receiving child-rearing allowance).
(2) Households in which the head of household is a government employee (civil servant).
(3) Households that have become a single-parent family due to divorce after September 2021 or have returned or entered Japan from overseas after September 2021 and are raising a child of high school age or younger*, etc.(This part has been newly added.)
*Household that have returned or entered Japan from overseas and are raising a child of high school age or younger: As to children of junior high school age or younger, they must have been returned or entered Japan on or after September 1, 2021 and for high school students, on or after October 1, 2021. Both the child and the person raising the child must have resident registration in Sapporo as of February 28, 2022.
※For more details, see 2-(3) Support for households who cannot receive Special Cash Payment for Households with Children due to divorce, etc. below.

 How to apply  ※For those who fall under 2. (1) and (2) above

Application documents have been sent from the City on January 12, 2022 to the households eligible for this special cash payment.

(1) Fill out the application form.
You can refer to the example with English translation.

(2) Make a photocopy of the passbook or bank card which you wish to receive the payment.
If you fall under the followings, additional document is needed.
(a) You are a goverment employee and have a child aged under junior high school:
 Make a photocopy of the document to prove that you have recieved the child-rearling allowance for September 2021.
(b) You have a child who are living separately:
 Make a photocopy of the resident registration (Jumin-Hyo) which includes all members of the household that the child belongs to.

(3) Put (1) and (2)  into the enclosed envelop, put the stamp (84 yen)  and post it. 
If you fall under one of the followings, you will need to contact  the Sapporo Call Center for Special Cash Payment for Households with Children.
(1) Households in which all the children do not have resident registration in Sapporo City as of September 30, 2021.
(2)Households in which the head of household is a government employee and all the children were born after October 2021.

  

 2-(3) Support for households who cannot receive Special Cash Payment for Households with Children due to divorce, etc. 

Those who are raising a child who has become eligible for the Special Cash Payment and have not received it yet may receive payment upon application.
The deadline for application is April 28, 2022 (Thu) *Must be postmarked by April 28.

 Eligibility    (Income restrictions apply)

Households that are raising a child of high school age or younger as of February 28, 2022 and fall under one of the followings:

A. Those who got divorced on or after August 31, 2021, are raising a child of junior high school age or younger, and did not receive a child allowance for September 2021 but became a recipient of the child allowance for March 2022.

B. Those who got divorced on or after September 30, 2021, were not a main supporter of a child of high shool age (born between April 2, 2003 and April 1, 2006) as of September 30, 2021 but have become a main supporter as of February 28, 2022.

C. Other persons in a similar situation (children of junior high school age or younger must fall under one of the followings on or after September 1, 2021, and high school students, on or after October 1, 2021).
・Persons who have not completed the procedures for DV victims, etc. to change the benefit recipients
・Persons who have become a foster parent through adoption
・Persons who have returned or entered from overseas and are raising a child of high school age or younger*
*As to children of junior high school age or younger, they must have been returned or entered Japan on or after September 1, 2021 and for high school students, on or after October 1, 2021. Both the child and the person raising the child must have resident registration in Sapporo as of February 28, 2022.

 How to apply  

(1) Make a phone call to the Call Center*(050-3850-1629)and tell them you want to apply for this special cash payment.

(2) You will be asked questions related to the application for the special cash payment. The City will send you an application package if you are eligible.

Please be aware of the points below.

  •  Public authorities will not ask for personal information in connection with the new coronavirus!
  • Don't open emails from unknown senders!
  • Don't initially trust the URLs in emails!
  •  Make sure you're on a legitimate site when you enter your ID, password, and credit card information!
  • If you receive something you don't recognize, refuse to receive it!
  • If you get a text message or a suspicious phone call from an unknown source, don't act on your own, always talk to the police or your family first!
